OUTSTANDING PAIN MENTORSHIP AWARD 2017
The award will consist of $1,000, a plaque, complimentary conference registration and a ticket to the annual conference dinner where the award will be presented.
This award will be presented annually at the CPS Annual Conference to a researcher and/or clinician who consistently exemplifies outstanding mentorship in the training of future pain researchers and/or clinicians.
A member of the Canadian Pain Society for at least 2 years who is not been a previous recipient of this award.
a. Candidates must be nominated by at least 1 colleague or current/former trainee. The person(s) nominating and the nominee must be members of the CPS.
b. Nominations must be made through our online form, and include the nominee’s curriculum vitae and a nomination letter (max 2 pages) describing the mentorship qualities of the candidate and signed by nominee(s). Note: All documents must be combined and uploaded into one PDF file. The file may not exceed 30 MB.
c. The complete nomination package must be received by the CPS no later than Monday October 31, 2016.
d. Selection Process: The selection will be made by a committee formed by the Canadian Pain Society.
