DISTINGUISHED CAREER AWARD 2017
The Distinguished Career Award recognizes longstanding contributions to pain research and/or management.
The award will consist of a Keynote Presentation at the CPS annual conference, economy class travel to the CPS conference, taxi fare, accommodation at the conference hotel for two nights, $1,000 honorarium, complimentary conference registration and a ticket to the annual conference dinner where the award will be presented.
Applicants must be a current member in good standing with the Canadian Pain Society. A letter of nomination signed by three CPS regular members, along with the CV of the nominee shall be submitted to the Secretariat through our online form. The letter of nomination should clearly summarize the nominee’s contributions to pain research and/or management (e.g., clinical responsibilities).
Note: All documents must be combined and uploaded into one PDF file. The file may not exceed 30 MB.
The selection will be made by a committee formed by the Canadian Pain Society.

Note: Travel details through the CPS office. Hotel accommodation will be made by the CPS. Receipts for travel to be sent to the CPS office within 14 days after the conference for reimbursement of expenses, plus the honorarium.
