SPECIAL OFFER TO CANADIAN ASSOCIATION FOR NEUROSCIENCE CONFERENCE ATTENDEES
The Canadian Pain Society is offering an incentive for any CAN member, who is attending the Canadian Association for Neuroscience meeting to attend the CPS Annual Conference at the member rates. The CPS Conference falls right behind the CAN meeting. The CAN meeting is in Vancouver, BC from May 20th to May 23rd and the CPS meeting is in Whistler, BC, beginning on May 23rd and ending on May 26th. To take advantage of this special incentive, choose the appropriate box on the registration form indicating that you are attending the CAN meeting and then you may then register using the published member rates – either by fax, or mail. Online registration for this category cannot be done—you have to register either by fax or mail. If you have any questions on this registration

ANNUAL DINNER, AWARDS AND ENTERTAINMENT EVENING—FRIDAY MAY 25, 2012
The Dinner, Awards and Entertainment evening is being held at the Hilton Whistler Resort and Spa on Friday May 25, 2012. To attend this event, you must purchase a dinner ticket for yourself and any guest(s). The cost of a dinner ticket has been subsidized by the Canadian Pain Society
in-part. The ticket purchase price before May 4, 2012 is $60.00 each. After May 4th and up to and including May 24th, the cost is $90.00 each. No tickets will be available on May 25th, 2012. See the registration form to order these tickets.
CHILD CARE POLICY
Children are permitted to attend the conference including poster and oral sessions provided the accompanying adult ensures that the child is quiet and appropriately behaved in order to avoid disruptions. A modest fee of $40.00, will be charged per child to cover the cost of food and beverage for breakfast, breaks and lunches. This does not include the annual dinner, or the reception, where children under the age of 18 are not permitted. Babysitting services can be arranged through the Concierge at the hotel, utilizing local babysitting services.

Guests are welcome to the conference and attend the Welcome Reception complimentary, and/or the Friday Dinner and Awards and
Entertainment evening (ticket purchase is necessary). Guests are not permitted to attend any of the sessions, or attend any of the breakfasts, refreshment breaks or luncheons. Please see the registration form to order tickets for the dinner.
HOW TO REGISTER
Register 3 ways—All fees are payable in Canadian Dollars only.
Online: www.canadianpainsociety.ca—Members will be provided with a special link to register online that enables them to register without having to provide and fill in all of their information i.e. address, organization, phone, etc. This information will automatically be pulled from the current membership listing. Once registered, the member’s information will also be logged into the Event Calendar available through the Members Only section. This section tracks attendance at all CPS events.

Payment can be made by cheque or credit card (AMEX, VISA, MasterCard). Please note: Credit card payments will be processed by Events In Sync, Inc. Your statement will show “Events In Sync, Inc.”. Events In Sync, Inc. is the managing company for the Canadian Pain Society.
Cancellations received prior to FRIDAY APRIL 20, 2012 are subject to a $60.00 administration fee. After Friday April 20th, there will be no refunds however delegate substitution can be made until FRIDAY MAY 4, 2012 with no substitution fee. After FRIDAY MAY 4, 2012 a $60.00 delegate substitution fee applies.
